Use your fan only in dry environments. Do not place fan in
bathroom, laundry area, or similar locations, or near sinks,
washing machines, swimming pools or other sources of water.
Never locate fan where it may fall into a bathtub or other water
container. Do not use outdoors. Do not use in damp environ-
ments such as flooded basements or garages.

DO NOT attempt to defeat any of these safety features or
instructions.
PLEASE READ BEFORE OPERATING

HOW TO USE:

• Plug fan directly into 120 volt AC wall outlet. Be sure plug

fits tightly in outlet. A loose fit will cause overheating and
damage to the plug. Always operate fan on its base in the
normal upright position.

• Set the selector switch to the desired position.

• To disconnect fan; turn switch off, then remove plug from outlet.

ASSEMBLY INSTRUCTIONS HV-21 (with 3 piece stand)

• Remove fan and base tube from carton.
• Place fan face down (nameplate down) on floor.
• Rotate the two short legs that are attached to the grille

into a straight up position.

• Position stand as shown in drawing, with rear of the

base tube close to the switch.

• Depress button on one leg, and slide the base tube onto

that leg. Make sure that the button is engaged in the
hole in the base tube.

• Depress button on the other leg, and slide the base tube

onto that leg. Make sure that the button is engaged in
the hole in the base tube.

• Failure to ensure that both buttons are engaged in the

base tube holes
can lead to in-
jury to persons
or damage to
property.

• Attach rubber

feet contained in
plastic bag to the
base tube per
the enclosed in-
structions.

• Plug the female

end of the power
cord in below the
controls.

• Set fan upright onto rubber feet, rotate to the desired

angle and plug the cord into a properly grounded
electrical outlet. Your fan is now ready to use.

Do not operate any fan with a damaged cord or plug or after the
fan malfunctions, has been dropped or damaged in any man-
ner. For repairs covered under warranty, see warranty state-
ment. For all other repairs contact Lakewood’s customer
service by telephone or mail for information and assistance.

Do not route cord under carpeting. Do not cover cord with
throw rugs, runner, or the like. Arrange cord away from
traffic area and where cord will not be tripped over.

To reduce the risk of fire or electric shock, do not use this fan with
any solid state speed control device such as a dimmer switch.

Do not insert or allow foreign objects to enter any ventilation
or exhaust opening as this may cause electric shock, and/or
a fire hazard.

To Avoid Overloading Motor: Do not leave fan where drapes or
curtains will get pulled into the fan or against the fan guard.

A fan has sparking parts inside. Do not use it in areas where
gasoline, paint, or flammable materials are used or stored. This
fan contains flammable materials and if ignited will support and
spread fires.

This appliance has a three-prong grounded plug that must be
inserted into a matching outlet. To reduce the risk of electric shock,
the grounding plug must not be cut off the plug. Where a two-
prong wall outlet is encountered, it must be replaced; contact a
qualified electrician. Do not use the three-prong plug with a two-
prong adapter. DO NOT attempt to defeat this safety feature.

Use your fan only with adequate wiring that is up to Code. Connect
to properly grounded outlets only.
