Thank you for purchasing the Böwe Bell + Howell Trūper 3200 Scanner. Please make sure that you include the
following items when sending in this form.
• A fully completed Rebate Form including the serial number(s) from the scanner unit(s)
• A copy of the sales invoice(s) showing the valid serial number(s)
• Completed W-9 Form for appropriate tax reporting (www.irs.gov). This is required only for the first unit sold.

• This rebate applies only to purchases of Böwe Bell + Howell Truper 3200 model scanner purchased between December 15, 2005 and March 15, 2006 from
United States and Canadian authorized resellers of Böwe Bell + Howell scanners. Refurbished, demonstration or any other models are not included in this
promotional offer. Specially priced Truper 3200 opportunities are ineligible for this promotion.
• This offer is extended to active members of the Böwe Bell + Howell Big Picture VAR program. If you have questions, please contact the Böwe Bell + Howell
inside sales team at 1-800-SCAN-494 to confirm your eligibility.
• Böwe Bell + Howell Scanner Division will be providing $300 checks directly to the Big Picture VAR sales person who sells the Truper 3200. Before the first one
is sent, a W-9 form must be sent in to Böwe Bell + Howell, at the address listed below.
• The Rebate Form must indicate a valid serial number(s). As proof of purchase, a copy of your sales invoice(s) must be included with your Rebate Form. Up to
four scanners can be included on a single form. If you have sold more than four Truper 3200 scanners, you may used multiple forms. You can also enter the
information on our website at http://www.bbhscanners.com/products/truper_rebate_form.html.
• All forms must be postmarked within 30 days of the purchase date, or by March 15, 2006, whichever comes first. Checks will be sent within two weeks of
receiving all necessary paperwork. The W-9 form must be completed in order for the first gift card to be sent.
• Böwe Bell + Howell is not responsible for items lost in the mail. Please keep copies of all the documents submitted.
• Claims that do not comply with the terms and conditions of this offer will be rejected. We regret that late submissions cannot be accepted.
