● Make one selection from each Table I and II using the column below the proper arrow.
● Select as many Table III options as desired (if no options or approvals are desired, specify 9X).
● A ( ● ) denotes unrestricted availability. A letter denotes restricted availability.
● Restrictions follow Table IV.
