The supply connections are labeled as follows (from top to bottom):
Chilled Water Return: Connection to chilled water return fitting on chiller.
Supply Water Outlet: This is to be plumbed to a drain
Chilled Water Discharge: Connection to chilled water discharge fitting on
chiller.
Supply Water Outlet: This is to be plumbed to a pressurized water line.
