in your correspondence:
• The make and model of your computer.
• System software and version number.
• Information on any other connected SCSI devices.
• The symptoms of the problem the computer is having.
• Messages that appear on the screen with the problem.
• The frequency of occurrence of the problem.
• Your name, address, and telephone or fax number.
• PCMCIA card type, manufacturer, and specifications.
