Complete and immediately mail the Product Analysis Card. We ask that you complete the
questionnaire for two reasons:
1. The US Center for Devices and Radiological Health requires that Panasonic keep a record of
installation of all installed optical laser equipment that can be supplied to them upon demand.
2. This data will be used to build a data base of end user application information for analysis that
will assist Panasonic in determining how better to provide customer support and for future
product enhancements.
IF YOU SHIP THE PRODUCT
Carefully pack and send it prepaid, adequately insured and preferably in the original carton. Attach
a letter, detailing the complaint, to the outside of the carton.
Do not send the product to the Executive or other Panasonic locations. They are NOT equipped to
make repairs.
