Check actuator stem for damage; replace actuator stem,

O-ring, and stem bushings, if necessary

Improperly assembled spring

Disassemble actuator and check cylinder and piston for

damage; reassemble actuator correctly

Internal valve problem

Refer to valve’s maintenance instructions
