Indicates a key input that requires you to hold
down KEY1 as you press KEY2.

Indicates a key input that requires you to hold
down KEY1 and KEY2 as you press KEY3.
PHDISK Commands, utilities, device drivers, directory
names, and file names appear in upper case.
