To create mark sense fields with box removal

Mark sense fields are used for tick boxes on a document. In some cases, a single tick
box will be used to indicate whether an item is required or whether a notice has been
read and understood. In this case, a single mark sense field can be defined. In other
cases, such as multiple choice questions, a number of tick boxes will be present, one
for each choice.

In general, if a group of tick boxes are arranged in a regularly spaced grid then a
single mark grid field may be defined for Recognition to cover all boxes. Completion
then provides a simple way of entering the data for a mark grid.
