Set vertical spacing to 6 lines per inch

Set form length to 33 (5.5 inches)

Select 10 cpi (characters per inch)

Set vertical tab stop for line 27


Line 16 executes the vertical tab \\11\\.

The last line executes the form feed command \\12\\.
