) If exposure has occurred, the customer must certify that decontamination has been
accomplished and that no radioactivity is detectable by survey equipment. Cleaning must be
certified on any product before returning it to YSI.
12-5. Packing Instructions and Product Return Form
Any system being returned for service must be shipped back in its original shipping container using
original packing material and all delivery cost to YSI covered by the customer (including all taxes
and duties). If the system is returned without appropriate packaging, the customer will be required
to cover the cost of a new packaging crate and material.
) Clean and decontaminate items to insure the safety of the handler.
) Complete and include the Product Return Form found on the following page.
) Place the product in a plastic bag to keep out dirt and packing material.
) Use a large carton, preferably the original, and surround the product completely with packing
material.
) Insure for the replacement value of the product.
